Het klinkt tegenstrijdig: het ene moment kun je niet naar het toilet, en het volgende moment heb je last van dunne, waterige ontlasting. Toch is dit een bekend fenomeen dat ‘overloopdiarree’ of ‘paradoxale diarree’ wordt genoemd. Het is geen ‘normale’ diarree, maar juist een direct gevolg van ernstige obstipatie (verstopping).
Wat is obstipatie (verstopping)?
Obstipatie, ook wel verstopping genoemd, is een veelvoorkomende klacht waar je je niet voor hoeft te schamen. Het betekent dat je stoelgang minder vaak komt dan voor jou normaal is, of moeizaam aanvoelt. Wat ‘normaal’ is, verschilt van persoon tot persoon: de één gaat drie keer per dag, de ander eens per drie dagen. Het gaat er vooral om hoe jij je voelt.
Wat is diarree?
Diarree is het tegenovergestelde van obstipatie. Je hebt last van dunne, waterige ontlasting die vaker komt dan normaal en moeilijk op te houden is. Meestal is het van korte duur (acuut), maar soms kan het langer aanhouden (chronisch).
Diarree ontstaat vaak plotseling en kan verschillende oorzaken hebben:
- Virussen: De meest bekende oorzaak is een virus, zoals bij een ‘buikgriep’.
- Voedselintoleranties: Een reactie op bepaalde voedingsstoffen, zoals lactose (in melkproducten) of fructose (in fruit en zoetstoffen).
- Medicatie: Sommige medicijnen, met name antibiotica, kunnen diarree als bijwerking hebben.
Het verband tussen obstipatie en diarree: overloopdiarree.
Het klinkt misschien vreemd, maar langdurige, ernstige obstipatie kan juist leiden tot een vorm van diarree. Dit noemen we overloopdiarree.
Hoe werkt dat precies?
Stel je voor dat er een harde, droge prop ontlasting vastzit in je darmen die er niet uit kan. Dit is de obstipatie. De nieuwe, vloeibare ontlasting die van boven komt, kan er niet langs en hoopt zich op achter deze blokkade. De druk wordt uiteindelijk zo hoog dat deze dunne, waterige ontlasting langs de randen van de verstopping ‘lekt’. Wat je dan ervaart, is plotselinge, oncontroleerbare diarree, terwijl de kern van het probleem – de harde blokkade – nog steeds aanwezig is.
Overloopdiarree is geen ‘gewone’ diarree, maar een duidelijk signaal van ernstige verstopping. De oplossing is dan ook niet om een diarreeremmer te gebruiken (dit kan het probleem verergeren), maar om de onderliggende obstipatie te verhelpen. Als je vermoedt dat je last hebt van overloopdiarree, of als je periodes van verstopping en diarree afwisselt, is het verstandig om contact op te nemen met je huisarts. Hij of zij kan de juiste diagnose stellen en je helpen de vicieuze cirkel te doorbreken.
